Job description
RESPONSIBLE FOR
To provide an accurate and timely dispensing/ preparation and supply services to patients and / or wards/ units which meets professional and legal standards.
SUMMARY OF DUTIES
The Pharmacy Assistant will be required to perform various tasks within the Pharmacy Services including dispensing, preparation of drugs, compounding, repacking and counseling of customers regarding drug delivery devices to ensure the safe and effective dispensation of medication to the provision of health care.
MAIN DUTIES RESPONSIBILITIES
ORGANIZATION AND MANAGEMENT
1. To understand and carry out pharmaceutical care in line with Vision and mission, Core Values, Goals and objective that suit the scope of the Pharmacy services and the hospital.
2. To understand the service organization structure.
3. To attend meetings held by the services in order to discuss issues and matters pertaining to the operation of the service.
4. To be the first liner in all dispensing duties which include:
Screening and order entry of prescriptions/ drug charts.
Confirmation with doctors in the event of illegible handwriting, dosage/ frequency/ duration error or drug not available.
Compounding pharmaceutical products for extemporaneous use.
Packaging, labelling, checking and dispensing of medications.
Sending prepared medications to the ward.
5. To conduct monthly ward check and pharmacy stock check on slow moving items, short expiry item and rotate stock if required. Report any discrepancies found to Pharmacist.
6. To provide patient counseling when required.
7. To provide drug information to patients, doctors, nurses, and other hospital personnel. Refer to Pharmacist if needed.
8. To restock drugs in pharmacy, wards and units and inform Pharmacist In-Charge of unavailability of drugs.
9. To ensure adequate inventory level of general supplies, e.g. prescription pads/ drug charts, disposables and dispensing items for smooth service operation.
10. To unpack the ward return medications.
11. To prepack bulk medications if required.
12. To accurately reconciling and finalizing discharge bill.
13. To prepare external solutions and disinfectants for ward/ clinics;
14. To participate in any stock take arranged by the hospital.
15. To compile daily and monthly statistics as per quality objectives.
16. To record and file Psychotropic Drug Prescriptions.
17. To ensure precision and accuracy while dispensing and report to Pharmacist or Pharmacist in-charge immediately in case of any error occurred.
18. Keep work area tidy and clean.
HUMAN RESOURCE AND DEVELOPMENT
1. Takes personal responsibility for own professional development and keep up to date with professional change and growth by maintaining mandatory learning hours.
2. To participate in the orientation of new staff in pharmacy.
3. Maintain the record of personal leave and training attended.
POLICIES AND PROCEDURES
1. Adheres to the hospital policies, procedure, guidelines and work instructions.
2. Adhere to all Acts, Laws, Rules and Regulation governing the pharmacy service at all time. Liaise and coordinate with Doctors and other services on issues related to Pharmacy Services
FACILITIES AND EQUIPMENT
1. To operate all equipment in the unit according to procedure and provides assistance/ guidance to new staff in their use.
2. To check that all equipment are functioning well and service according to PPM schedule.
3. To report to Engineering personnel if any faulty and takes necessary action accordingly.
4. To assist in the maintenance of equipment by proper cleaning, handling and utilization as directed.
5. To assist the pharmacist in keeping accurate records of equipment and drugs in the pharmacy
ENVIRONMENTAL, SAFETY AND QUALITY IMPROVEMENT ACTIVTIES
1. To carry out duties as an employee as stipulated in Occupational Safety and Health Act and Regulations 1994:-
- To take reasonable care at work for the safety of yourself and other persons;
- To co-operate with your employer or any other person in the discharge of any duty;
- To wear or use any protective equipment or clothing provided by the employer.
- To comply with any instruction or measure on occupational safety and health.
2. Participates actively in any other new service that is introduced by the services.
3. Participates in 5S activities and comply with the specified requirements.
4. Practices Standard People Practice (SPP) at all times.
5. Ensure compliance of Patient safety goals as safety improvement measures recommended by National Patient Safety Goals.
6. Policies on patient and family rights are addressed at all times.
7. To participate actively and encourages compliance to all policies and Procedures of ISO9001:2015, ISO 14001:2015 and OHSAS 18001:2007, HACCP Netherlands, Malaysian Hospital Accreditation Standards, Joint Commission International Accreditation Standards (JCI) and NPSG.
ADDITIONAL DUTIES AND RESPONSIBILITIES IN THE SPECIFIC AREAS
1. To be involved in various committees/ activities assigned by management.
2. To be a preceptor to Diploma in Pharmacy Students and to attend any preceptor courses assigned. (if applicable).
3. To be a mentor to train and guide new Pharmacy Assistants.
4. To perform any other duties as ordered by the Pharmacist/ Pharmacist In-Charge.
QUALIFICATIONS REQUIRED
- Minimum: Diploma in Pharmacy
- Preferred: Able to communicate fluently and write in English language and Bahasa Malaysia
EXPERIENCE REQUIRED
- Should demonstrate evidence of training in hospital setting during his/her undergraduate Diploma.
Proficient in common computer applications
Ability to check and detect errors during dispensing of medications.
