Job description

Responsibilities

  • Assist residents and patients with activities of daily living, including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts.
  • Administer prescribed medications accurately following all safety protocols and documentation procedures.
  • Monitor vital signs and observe changes in health status, promptly reporting findings to nursing staff.
  • Maintain detailed medical records and documentation using electronic health record (EHR) systems like Epic or Cerner to ensure compliance with HIPAA regulations.
  • Support specialized care needs such as dementia, Alzheimer's, memory care, behavioral health, hospice, or post-acute care services.
  • Follow aseptic techniques during procedures like catheter care or phlebotomy to prevent infection and ensure patient safety.
  • Collaborate with interdisciplinary teams in fast-paced environments including hospitals (ICU, emergency medicine), nursing homes, assisted living facilities, or outpatient clinics to deliver comprehensive patient care.

Skills

  • Proven experience with EMR/EHR systems such as Epic, Point Click Care, Cerner, or eClinicalWorks for accurate medical documentation.
  • Strong knowledge of medical terminology, physiology, anatomy, and vital signs monitoring.
  • Experience working in diverse settings including long-term care, acute care hospitals (Level I & II trauma centers), pediatric units (PICU), or behavioral health facilities.
  • Skilled in medication administration and understanding of HIPAA compliance standards.
  • Ability to perform complex caregiving tasks such as catheter care, aseptic technique procedures, and use of assistive devices like Hoyer lifts.
  • Familiarity with specialized care areas including dementia/memory care, infectious disease management, hospice support, or geriatrics.
  • Excellent communication skills combined with a proactive attitude toward patient service and teamwork in high-pressure environments such as ICU or urgent care settings.
