Description:

Position Summary

The Community Health Worker (CHW) is a frontline public health professional who serves as a liaison between the Grayson County Health Department and the community. The CHW works to improve health outcomes by providing health education, assisting residents in accessing healthcare and social services, and supporting individuals and families in navigating local health systems.

This position focuses on addressing barriers to care in rural communities, promoting healthy behaviors, and connecting community members to available resources and services.

 The CHW must meet all documentation, service delivery, and supervision requirements necessary for reimbursement under applicable Kentucky guidelines.

Essential Duties and Responsibilities

1. Community Outreach & Engagement

  • Conduct outreach activities to increase awareness of health department services and community resources.

  • Build trusting relationships with individuals, families, and community partners.

  • Participate in community events, health fairs, and public health initiatives.

2. Health Education & Support

  • Provide culturally appropriate health education on topics such as:

  • Chronic disease prevention

  • Maternal and child health

  • Mental health awareness

  • Nutrition and physical activity

  • Promote preventive health services including screenings, immunizations, and wellness visits.

  • Support self-management of chronic conditions.

  • Promote preventive care and appropriate use of healthcare services.

3. Resource Navigation and Referrals

  • Assist individuals and families in identifying and accessing healthcare, behavioral health services, and social support programs.

  • Provide referrals to community resources such as food assistance, transportation, housing services, and insurance enrollment programs.

  • Follow up with clients to ensure successful connections to services.

4. Care Coordination and Support

  • Support individuals in scheduling appointments and understanding healthcare recommendations.

  • Help residents overcome barriers to accessing care, including transportation and health literacy challenges.

  • Provide basic support and guidance to help individuals manage chronic health conditions.

5. Data Collection and Documentation

  • Maintain accurate records of outreach activities, client interactions, and referrals.

  • Collect program data for reporting and evaluation purposes.

  • Assist with community health assessments and program evaluations.

  • Demonstrate and follow billing guidelines.

6. Collaboration

  • Work collaboratively with health department staff, healthcare providers, schools, faith-based organizations, and community agencies.

  • Participate in team meetings and training opportunities.

7. Grant Compliance & Reporting

  • Collect and report required program data in accordance with grant requirements.

  • Track measurable outcomes and performance indicators.

  • Assist with program evaluation and quality improvement activities.

  • Participate in audits and monitoring activities as required.

8. Documentation & Confidentiality

  • Document all client interactions timely and accurately in approved systems.

  • Maintain compliance with HIPAA, Medicaid regulations, and local health department policies.

  • Protect confidentiality of all client records and communications.

Minimum Qualifications

  • High school diploma or GED required.

  • Kentucky Community Health Worker Certification (or ability to obtain within 3 months of hire date).

  • Experience in community outreach, healthcare, social services, or public health preferred.

  • Valid driver’s license, reliable transportation, and vehicle insurance.

  • Ability to pass required background checks.

Preferred Qualifications

  • Experience working with rural populations.

  • Familiarity with electronic health record systems.

  • Knowledge of Kentucky billing requirements for CHW services.

Knowledge, Skills, and Abilities

  • Strong interpersonal and relationship-building skills.

  • Cultural competence and sensitivity to diverse populations.

  • Ability to work independently in community settings.

  • Organizational and time-management skills.

  • Basic computer proficiency and data entry skills.

  • Understanding social determinants of health.

Physical & Work Requirements

  • Travel throughout Grayson County is required.

  • Ability to attend community events.

  • Occasional evening or weekend hours may be required.

  • Combination of office, community, and field-based work.

  • Ability to lift educational materials or supplies up to 25 pounds if needed.

Supervision

This position will operate under the supervision of a qualified healthcare professional as required for Medicaid billing compliance. Regular supervision meetings and documentation review will be required.
