Speech Therapist (SLP-CCC)

This is an in-house Therapy Team.  Join us and work directly for the center, not via a contract company.  With the support of our discipline specific regional therapy managers, you will be a part of our first in-house team. 

Our culture is a fast-paced environment and we're looking for a passionate Speech Language Pathologist / Speech Therapist (SLP-CCC), with innovative approaches, and expertise in the care they give.  In this SNF, your caseload will be a combo of Med A, Med B & Public Aid patients.  In other words, some cognitive, some language and a few swallowing/dysphagia.

Speech Language Pathologist / Speech Therapist (SLP-CCC)

What we need from you:

• * Speech Language Pathologist / Speech Therapist (SLP-CCC) license in IL
• * Experience with Med A, Med B & public Aide patient caseload
• * Experience in a post-acute/inpatient rehab setting is helpful
• * Technologically savvy — we use PCC & Optima

This center is managed by TRANSITIONAL CARE MANAGEMENT.  We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
