Nurse Navigator - Cancer and Infusion Center

A Day in the Life 

• Act as a liaison between patients, professional staff and physicians by providing care management and coordination for adult and pediatric patients with complex conditions, social needs and educational needs.

• Serves in an expanded health care role to collaborate with primary care providers, specialists, and patient/families to ensure the delivery of quality cand cost-effective health care services.

• Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status and decreasing hospital and emergency room utilization.

• Support quality improvement initiatives and adherence to safety standards.

What’s Required  

• Licensed RN in the State of Michigan 

• Experience in complex chronic disease management preferred 

• Bachelor of Science degree in nursing preferred

• Five years' (5) experience as an RN preferred
