As a Medical Social Worker with Best Home Health Providers, you will shape how we care for patients in their homes, mentor a team of social workers, and help set the standard for compassionate, high-quality service across our organization. Please note that this role is per diem with a rate of $100 - $120 per visit. Responsibilities Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards Partner with the Executive Director on the annual department budget and resource planning Build strong knowledge of community resources and connect patients and referral sources to the right support Plan in-service training and continuing education to keep the team’s skills sharp Contribute to quality improvement initiatives and help identify new ways to better serve our patients Perform other duties as required for patient services
A Master of Social Work (MSW) from an accredited program An active California license as an LCSW At least 3 years of management experience, preferably in home care Strong knowledge of healthcare regulations, ACHC standards, and business operations Excellent communication skills and the ability to build trust with patients, families, staff, and the community A self-directed, organized, and flexible approach—you take initiative and work well with others
