We’re seeking an Activity Aide to involve residents in a therapeutic engagement program. You will help residents with life enrichment activities and a variety of entertainment options to help keep them engaged in their recovery programs. We are seeking candidates with a strong background in sports, culture, music, and traditional recreational activities, to help bring joy, structure, and stimulation to those in our care.
What You’ll Do
- ✅ Lead engaging group events, ranging from fitness and games to music and arts
- ✅ Lead stimulating individual cognitive and memory retention activities for our long-term residents
- ✅ Maintain accurate documentation in PointClickCare (EMR system)
- ✅ Collaborate with our health care teams to individualize your approach
- ✅ You may be asked to drive and/or escort residents to off-site events, appointments or activities utilizing our facility van

Activities Aide (in other centers this may be called a Life Enrichment Coordinator)
- Prior experience as an Activity Aide in a Skilled Nursing Facility (SNF), Intermediate Care Facility (ICF), or Assisted Living Facility (ALF) is strongly preferred.
- The ideal candidate is outgoing, compassionate, and skilled at creating a welcoming environment that encourages resident involvement and participation.
- Experience with electronic medical records (EMR) or patient documentation systems is preferred.
- If assigned transport (driving) responsibilities, must maintain a valid driver’s license and meet the organization’s motor vehicle record and insurance eligibility requirements.
- Must possess a high school diploma or equivalent (GED).

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
