We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.

🌟 The Role

You’ll deliver person‑centred, trauma‑informed, strengths‑based case management that reflects TSA’s mission and values. Working as part of a collaborative team, you’ll ensure services align with national and state strategies and the National Homelessness Stream Model of Care.

🔑 What You Will Do

  • Build strong, trust‑based relationships that encourage client participation and self‑determination
  • Provide case management in line with TSA models and practice standards
  • Complete thorough assessments of needs, risks, and vulnerabilities
  • Conduct ongoing reviews from intake through to exit
  • Develop collaborative case plans with clear goals, interventions, and pathways
  • Support clients to access services, community supports, and advocacy where needed
  • Monitor progress, review outcomes, and adjust plans to meet evolving needs
  • Maintain accurate, up‑to‑date client records using TSA systems
  • Identify risks and escalate promptly to supervisors
  • Recognise service gaps and contribute to service improvement
  • Participate in network meetings, Communities of Practice, and development activities
  • Demonstrate emotional intelligence and self‑awareness in practice
  • Engage in regular supervision, case reviews, and professional development
  • Adapt to change and respond effectively in dynamic environments
  • Participate in on‑call roster and be available for night work

🎓 What You Will Bring

  • Degree in social work, welfare, community development, or related field
    * A degree qualification is preferred
  • Experience in social services, including assessment and case management
  • Strong understanding of trauma‑informed and strengths‑based practice
  • Current QLD Employee (Paid) Blue Card (WWCC)
  • Current Driver’s Licence
