PRN (Per Required Need) / On-Call Position
This is an as-needed, on-call role with no guaranteed hours.
Minimum Availability Requirements:
- Commitment to at least two (2) weekend shifts per month (Saturday and/or Sunday)
- Intermittent weekday availability as needed
- Flexibility to work Day, PM, and NOC shifts (PMs and NOCs are most needed)
- Availability to work some holidays
- Availability to attend a Wednesday New Employee Orientation
Additional Details:
- Shifts may be scheduled in advance or offered with short notice
- No guaranteed hours
- No benefits or insurance offered for PRN status
- Non-union position
Floor Nurse: We will consider a Licensed Practical Nurse (LPN) for this role.
What we need from you:
- Licensed Practical Nurse (LPN) license in IL
- Prior floor nurse experience within a skilled nursing facility is strongly preferred
- Experience with the post-acute patients – specifically the cardiac, post op ortho & the medically complex
- Electronic medical records experience is helpful (Point Click Care or PCC is ideal)

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
