Job description
We are seeking a compassionate and skilled RN (Registered Nurse) or LPN (Licensed Practical Nurse) to join our home health care team in Concord, United States. As a Home Health Nurse, you will play a crucial role in providing high-quality care to patients in the comfort of their own homes.
- Conduct initial and ongoing patient assessments to develop and update care plans
- Provide direct patient care, including wound care, medication administration, and vital sign monitoring
- Educate patients and their families on disease management, medication, and proper care techniques
- Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care
- Accurately document patient conditions, treatments, and progress in electronic health records
- Manage and organize daily visit schedules to maximize efficiency
- Adhere to all healthcare regulations and maintain patient confidentiality
- Participate in quality improvement initiatives and ongoing professional development
- Current RN license (Bachelor's or Associate's degree in Nursing) or LPN license (completion of an accredited practical nursing program) in the state of practice
- Valid CPR certification
- Strong clinical skills and knowledge of medical equipment and procedures
- Excellent communication and interpersonal skills
- Proven ability to work independently and manage time effectively
- Proficiency in electronic health record systems and documentation
- Experience in home health care preferred
- Understanding of home health care regulations and best practices
- Knowledge of common chronic conditions and their management
- Ability to adapt to various patient populations and home environments
- Valid driver's license and reliable transportation
- Additional certifications (e.g., IV therapy) may be required
