Job Details Job Duties: Assess, plan, develop and provide the patient or responsible caregiver a home health plan of care for occupational therapy. identifies patient needs for other disciplines and services. Coordinate and evaluate the plan of care with all other disciplines, organizations and physicians for an assigned caseload. Communicates significant changes in patient status to the physician, rehab coordinator and the appropriate agency personnel when indicated. Demonstrates effective communication skills, keeping lines of communication open between Lead O.T. Supervisor, Team Managers and all staff involved in care of assigned patients. Minimum Requirements: 2 years of experience as an occupational therapist Current registration to practice Occupational Therapy in New York State Salary Range USD $90,000.00 - USD $115,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
