Position Overview:
We are seeking a passionate and organized Rehab Lead to oversee the eligibility assessment and coordination of our rehabilitation program. This role will involve working closely with residents, their families, and interdisciplinary teams to ensure high-quality care and support.
Primary Duties:

- Assessment and interpretation of client information for the purpose of determining eligibility for rehab program at the home based on Admission Criteria
- Accurate collection of information relative to the clients’ condition, assuring proper equipment is in place to support the participant’s physical requirements.
- Managing the coordination of and participating in the timely completion of Resident assessments (initial RAI and prior to discharge), tracking forms, Resident Assessment Protocols (RAPs), Resident Care Plans.
- Accurately assess client needs and coordinate rehab program services to support needs
- Support and mentor staff assigned to Rehab Program including Registered, PSW, Recreation, and Social Service Worker.
- Identify educational opportunities to enhance staff development.
- Effectively maintain required documentation including comprehensive care plans, assessment plans and incidental charting.
- Collaborate with other managers of the home to ensure adequate stock, supplies, lab service and communication of program support needs.
- Responsible to meet Ministry guidelines for the coordination of the rehab program.
- Responsible for quality monitoring and quality improvement activities that are based on outcome measures and resident satisfaction.
- In consultation with management and interdisciplinary team members, developing processes that ensure data integrity, efficient data collection process, assessment completion and data utilization.
- Maintaining and measuring indicators to support program improvements.
- Other duties deemed appropriate by the Manager, Resident Care related to the rehab Program
- Flexibility in working hours.
- Physiotherapist Therapist Diploma from a recognized college or university preferred
- Holds certification for ADP assessment & authorization an asset
- Kinesiology certification from a recognized college or university
- Registered Practical Nurse with a current registration and in good standing with the College of Nurses of Ontario
- Experience in geriatric nursing with a genuine interest in all aspects of Long Term Care – with a strong understanding and knowledge of Nursing Rehab, Restorative and Rehabilitative Care
- Holds certification in Restorative Care from the Canadian Centre for Activity and Aging (or can provide proof that you are enrolled in the course)
- Previous experience working in Long Term Care in this role (i.e. at least 3-5 year experience)
- Must possess excellent communication and problem solving skills
- Must possess clinical documentation skills (i.e. PCC, rehab coding, excel, word, etc.)
- Demonstrated success leading new initiatives
- Excellent interpersonal, communication, organizational and team skills
- Ability to speak to a group as needed
- Ability to work independently and strong problem solving skills
- Well-developed oral and written communication skills
- Well-organized, time management skills and an aptitude for detail-oriented work
- The ability to work with and relate to interdisciplinary team members, internal and external stakeholders
- Knowledge of LTC legislation, MOH compliance and Health and Safety
- Proficient in HQO principles and activities related to quality improvement activities including data collection, analysis, reporting, building QIPS
