The Community Health Worker Apprentice is a key role focused on developing the essential skills needed to serve as a liaison between patients, the healthcare system and community-based organizations. Working under the mentorship and guidance of an experienced CHW preceptor, the apprentice will gain hands-on experience and training across a variety of functions aimed at improving health outcomes for diverse and underserved populations. In this role, you will help build strong community partnerships, promote health equity and support access to vital health and community services.

A caregiver in this role works Monday through Friday from 8:30 a.m. to 5:00 p.m. between August 17 and September 20, including approximately 20% local travel within Northeast Ohio.
To be considered for this position, candidates must reside in Northeast Ohio.

You will attend Tri-C and work concurrently. Classes are held on Monday, Tuesday, and Thursday from 6:00 p.m. to 9:00 p.m. The format is hybrid (Zoom + Metro Campus onsite labs).

  • Location: Virtual via Zoom and Manufacturing Technology Center for onsite labs

  • Apprenticeship location: Langston Hughes

After successfully completing the Tri-C program and passing the required exam, you will transition into a Community Health Worker role with us at the Main Cleveland Clinic campus.

Beginning September 21 (training schedule), the hours are:

  • Monday & Tuesday: 8:00 a.m. – 4:00 p.m.

  • Wednesday: 8:00 a.m. – 3:30 p.m.

  • Thursday: 8:00 a.m. – 4:30 p.m.

  • Friday: Off

A caregiver who excels in this role will:  

  • Assist patients in navigating the healthcare system, including scheduling appointments and connecting them to services.

  • Provide education on available health resources, programs and preventive care services.

  • Support outreach efforts to engage diverse and underserved populations in the community.

  • Build and maintain relationships with community-based organizations and service providers.

  • Help identify social determinants of health needs and connect patients to appropriate resources.

  • Document patient interactions and maintain accurate records in designated systems.

  • Participate in care team meetings and collaborate with healthcare providers to support patient needs.

  • Assist with care coordination and follow-up to ensure continuity of services.

  • Support health promotion activities and community events.

  • Maintain confidentiality and comply with all organizational policies and procedures.

  • Complete training and learning objectives under the guidance of a CHW preceptor.

Minimum qualifications for the ideal future caregiver include:  

  • High School Diploma or GED

  • Knowledge and insight to effectively support the needs of patients and families

  • Strong time management and organizational skills, with the ability to prioritize tasks efficiently and manage multiple responsibilities

  • Effective de-escalation, problem-solving and conflict resolution skills, with the ability to adapt to change

  • Proficient in basic computer software and systems

  • Strong communication and information-gathering skills, including effective interviewing techniques

  • Excellent interpersonal skills with special emphasis on oral and written communication skills

  

Preferred qualifications for the ideal future caregiver include:

  • Bilingual in Spanish

  • Community service experience

  • Past work experience in a medical office setting

  • Informal community health worker experience

  • Epic experience

  • Healthcare experience

The apprenticeship flat rate is $18.50 per hour.

Physical Requirements:

  • May need to move around intermittently during the day, including sitting, standing, stooping, bending and ambulating. May need to remain still for extended periods, including sitting and standing. 

  • Evening and weekend work may be required.

  • May need to lift boxes or carry objects.

  • Possess dexterity to operate a PC and other office equipment.

Personal Protective Equipment:

  • Follow Standard Precautions using personal protective equipment as required.
