The Community Health Educator (CHE) who will serve as the primary coach, educator, and navigator for SCAN members enrolled in the Food is Medicine (FIM) pilot program; not just nutrition education, but also address broader SDOH needs. Working telephonically, the CHE delivers culturally responsive, evidence-based health education centered on practical nutrition improvements, grocery shopping guidance, and whole-person wellness, with deep attention to the social, economic, cultural, and functional realities of each member. The CHE also contributes to program evaluation, outcome tracking, and continuous improvement efforts to assess program impact and effectiveness.

You Will

Member Outreach and Engagement: Conducts outbound and inbound outreach to members who are enrolled in or eligible for the Food is Medicine Pilot. Introduces the program in a clear, warm, and welcoming way that encourages participation. Builds rapport and trust through respect, person centered communication. Helps members understand the value of the program by clearly explaining “what’s in it for them” and how it can support their nutrition needs, health priorities, and personal goals.

Assessment and Care Planning: Conducts structured, person-centered assessments to understand nutrition needs, priorities, and readiness for change. Identifies barriers that impact healthy eating and overall well-being. Stratifies members based on individual needs and risk factors, and partners with members to develop realistic, achievable goals that reflect what matters to them and support sustainable behavior change.

Health Education and Coaching: Provides simple, practical support around food and healthy eating. The CHE help members make small, realistic changes based on what feels right for them. Education and support are tailored to the member’s daily routine, culture, preferences, and any challenges they may face, with ongoing encouragement to support their wellness goals.

Resource Navigation and Connection: Connects members to appropriate internal or community based resources that align with their needs and priorities. Identifies urgent or complex concerns and escalates them promptly to the appropriate SCAN team, or supervisor in accordance with established workflows and protocols.

Program support and Documentation: Documents all member interactions in a timely and accurate manner, including goals, identified barriers, education provided, and agreed-upon next steps in the appropriate systems. Track outreach attempts, engagement status, schedules follow-up interactions based on their established cadence for each member’s tier level and follow-up needs. Supports pilot workflows, reporting requirements, and ongoing process improvement efforts. Actively participate in team meetings, training, and quality improvement activities to support program success.

Member Experience: Delivers a supportive, respectful, and encouraging experience during every member interaction. Uses clear, plain, and senior-friendly language to ensure understanding. Fosters confidence, empowerment, and shared accountability while maintaining a non-judgmental compassionate approach.

Your Qualifications

Experience Required: 2-3 years of related experience in two or more of the following:

• Health education

• Community outreach

• Wellness coaching

• Care coordination support

• Public health

• Diabetes prevention or lifestyle coaching

BILINGUAL- Must be bilingual in English/Spanish. (Test will be administered to assess proficiency.) 

High School Diploma or equivalent experience

Technical expertise - Basic technical skills for functional area

Active listening skills

Motivational interviewing and coaching experience

Problem-solving and person-centered communication

Cultural humility

Data-Driven Approach: Proficiency in using data and member feedback to evaluate the effectiveness of health education programs and make informed improvements.

Strong Communication Skills: Ability to effectively communicate complex health information in a clear and accessible manner, both verbally and in writing, to diverse member populations.


Health Education Expertise: Knowledge of health education principles, methods, and best practices, with a focus on disease management, preventive care, and health promotion.
