Job Overview Ebenezer is seeking a RAI Coordinator for our Skilled Nursing Community in Wyoming, Minnesota! This Meadows on Fairview TCU nursing role is responsible for coordinating and ensuring the accuracy of the Resident Assessment Instrument (RAI) and Minimum Data Set (MDS) processes. This role ensures compliance with state and federal regulations, supports optimal reimbursement, leads MDS scheduling, and collaborates with interdisiplinary team members to develop comprehensive care plans. The RAI Coordinator reports to the Director of RAI and plays a critical role in quality improvement and accurate clinical documentation. This Nursing schedule includes; Casual / on call Day shift Weekends as needed Meadows on Fairview TCU is located in the city of Wyoming, MN, steps away from the Fairview Lakes Hospital and a short distance from Forest Lake and North Branch. We offer a home like atmosphere where you feel like you belong the minute you arrive! Our 14-bed Transitional Care Unit is small enough to get to know your co-workers and residents yet large enough to provide you with opportunities and room to grow.
