$38.20 - $57.30Where You Will Work:
CHG Cabarrus is a hospitalist medicine group embedded within the hospital setting, providing comprehensive inpatient care and care coordination for patients throughout their stay. Our team works closely with case management, nursing staff, pharmacy, and consulting physicians to facilitate safe and timely discharges. The Nurse Clinician serves as a key liaison between CHG physicians, hospital staff, patients, and families, supporting discharge planning, patient communication, and transitions of care.
We’re Looking For:
- Minimum 2 years of RN experience in an acute care hospital setting, ED or ICU
- Experience with discharge planning, case management support, or transitions of care
- Familiarity with EPIC
- Bachelor of Science in Nursing (BSN)
Minimum Requirements
- Associate Degree in Nursing with active RN licensure in NC
- Active and Current BLS certification
What You’ll Do:
- Works with provider(s) to create focused patient-centric discharge plans, addressing barriers to discharge for patients with length of stay (LOS) that is longer than expected
- Coordinates the evaluation, testing, management and follow-up of patients in collaboration with the attending physician, and other members of the healthcare team
- Rounds with physicians and serves as a liaison between physician, healthcare team, patient and family members
- Promote and assist with patient and family education supporting the treatment plan and educational needs of the patient/family members to achieve continuity of care as care transitions to the home environment or next level of care
- Ensure patients have scheduled follow up appointments prior to discharge
