Job Description:

SHIFT: *This role requires travel and will drive a company vehicle (small bus) throughout the communities in the area M-F

  • Connects with patients prior to discharge to understand contributing factors and barriers, identifying social and resource needs.
  • Educates patients and families about available community resources, programs, and social services.
  • Conducts post-discharge phone calls to provide support and encouragement, obtain updates on recovery progress, and collaborate with resource providers for additional assistance.
  • Demonstrates good interpersonal communication skills in challenging situations, interacting respectfully, calmly, and courteously with patients, physicians, colleagues, and others.
  • Identifies patient needs and helps find resources to address those needs. Works independently and collaboratively with a team, showing flexibility.
  • Utilizes active listening skills to understand patient concerns and needs.
  • Delivers health information using culturally appropriate terms and concepts. Demonstrates effective organizational skills, collecting and tracking data, prioritizing tasks, handling multiple responsibilities simultaneously, and maintaining attention to detail.
  • Uses computer applications such as Microsoft Office Suite (Outlook, Word, Excel, PowerPoint) efficiently. Other duties as assigned.
  • Helps patients schedule appointments with resource providers and other necessary services.
  • Assists patients with navigating insurance applications, verification, and authorization, as well as counseling services and other resource providers tailored to their social needs.

Knowledge, Skills, and Abilities:
• N/A

Education:
• High School Grad or Equiv [Required]

Field of Study:
• graduate or equivalent
• Community Health Worker Training/Certification

Work Experience:
• Experience in a clinical or community health setting [Required]
• Experience in community education, public health, community outreach, social service, community advocacy or education [Preferred]
• Experience working with people of different cultures and socioeconomic backgrounds [Required]
• Insurance authorization/verification experience [Preferred]
