Job description

Supervising all hands on care but not completing hands on care. Duties include but are not limited to setting up charts, arranging for in home services and transportation, and overseeing patients’ daily lives (medication and physician visits).

  • Must have RN licensure
  • Homecare Nurse Supervisors are a great fit for this position!
  • Reliable transportation since this position will require traveling to members homes
  • Provided case management services to patients with chronic or complex conditions
  • Proactively identified members that may qualify for potential case management services.
  • Conducted assessment of patient needs by collecting in-depth medical information

Hours for this Position: Monday-Friday 8:00am-5:00pm
