Community Health Worker serves as a liaison between health/social services and the community. The Community Health Worker will work with clients throughout the PowerBP+ intervention; they will work helping individuals, families, groups and communities develop their capacity and access to resources, including health insurance, food, housing, quality care and health information. will be a link for patients through Phase I and Phase II of the program, and will also team with IPE students to extend the CHW reach, and to expose students to the role of CHWs on healthcare teams. The CHW will address social needs through resource referral and connection with the Pathways Community Hub Model.
