Job Highlights:

The Dean Medical Group Anticoagulation Clinic provides expert management of anticoagulation therapy for our patients, earning recognition as an Anticoagulation Center of Excellence. Our team of clinical pharmacists practice under a collaborative practice agreement to manage warfarin, DOACs, and perioperative anticoagulation plans. Clinicians oversee dedicated patient panels to foster strong relationships and continuity of care, collectively managing ~2,600 warfarin patients, 3,900 DOAC patients, and 200 perioperative plans monthly. Operating fully remotely, we partner closely with primary care, cardiology, and surgical teams and leverage telephone and MyChart visits to deliver efficient, high-quality care for patients across more than 60 clinics in south-central Wisconsin.

Schedule:

  • The 0.5 FTE (20 hours per week) schedule will primarily be Mondays, Fridays, and every other Wednesday – although this will be somewhat flexible and depend on the needs of our team

  • Hours: 8:30am-5:00pm, no evenings, weekends, or holidays

  • Location:

    • Training will take place at the SSM Wisconsin Regional office located at 1808 W. Beltline Hwy, Madison, WI 53713

    • Once fully trained, this position will be 100% remote. Candidate must be located in Wisconsin.  

Job Summary:

Assumes an active role to provide patient care through comprehensive medication reviews and/or targeted disease-specific interventions.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Gathers necessary data to promote patient safety by ensuring that each relevant medication is indicated, effective, safe, and that the patient is adherent to therapy
  • Improves patient-specific drug therapy and therapeutic problems by evaluating the appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects.
  • Assesses relevant patient-specific medical problems, prioritizes medical problems based on urgency and severity, and identifies preventive and health maintenance issues as needed. May design a drug therapy plan for patient-specific problems and persuasively justifies recommendations.
  • Provides enhanced patient care through detailed counseling on medications and disease states, adherence assessment, medication cost analysis, review of immunization status, and communication with patient’s, providers, and pharmacy.
  • Provides consultation to other health care professionals if requested.
  • For Specialty Pharmacy Only: Conducts patient counseling and monitoring as required by the patient management program.
  • For Specialty Pharmacy Only: Participates in the quality management program to facilitate continuous process improvement initiatives and quality improvement projects.
  • Precept pharmacy students and residents, as applicable.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.


EDUCATION

  • Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacy

PHYSICAL REQUIREMENTS

  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
  • Frequent keyboard use/data entry.
  • Occasional bending, stooping, kneeling, squatting, twisting and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Rare climbing.

REQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS 

  Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment. Other resuscitation certifications may be required dependent on department.
