A caregiver in this role will serve patients in the general Cleveland area.
Cleveland Clinic’s Home Care team helps patients continue their treatment and learn/relearn any skills necessary to stay safe and independent again. As an RN, you are responsible for providing direct nursing care, participating in a team-based care model, implementing and monitoring patient care plans, and reporting patients' needs after every visit. Due to continuously working with each patient, Homecare RN’s have more acute findings for patients and are easily able to identify changes. A caregiver in this position works 5 8-hr shifts from 9:00am to 5:00pm. Requirements include 1 winter and 1 summer holiday and 1 on-call shift per week. Weekend requirements occur every 4th weekend.
A caregiver who excels in this role will:
- Provide direct nursing care.
- Perform accurate and complete admission assessments and reassessments within a specified time frame.
- Assess patients for safety risks, abuse and neglect.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care.
- Assess, document, prioritize and manage patients’ pain.
Minimum qualifications for the ideal future caregiver include:
- Graduate from an accredited school of Professional Nursing
- Current state licensure as a Registered Nurse (RN)
- Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross
- Valid Ohio Driver's (VDL) license upon hire and per policy 8-175 thereafter
- Proof of $100,000/$300,000 automobile insurance coverage upon hire and per policy 8-175 thereafter
- One year of current experience as an RN
- One year of experience in an acute Medical/Surgical or Home Care setting
Preferred qualifications for the ideal future caregiver include:
- Bachelor’s of Science in Nursing (BSN)
- Familiarity with complex medical treatments
- Experience working in homecare, med/surg, ICU and/or ED areas

Physical Requirements:
Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
Must be able to read, write and communicate with patients.
Must have correctable vision and correctable hearing to be able to hear normal conversations. Must be able to work in settings with infectious diseases.
Must present a professional appearance at all times and attend staff and agency meetings, as requested.
Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.
