Job description
1) GENERAL DESCRIPTION
Responsible for coordinating, monitoring, and supervising the operational, administrative, and quality activities of the Health Information Management Services (HIMS) Department. The position supports the Head of Services in ensuring effective management of medical records, medical reports, clinical coding, statistics, data governance, and regulatory compliance.
2) JOB DESCRIPTION
Assist the Head of Services in overseeing the daily operations of the Medical Records, Medical Reports, Clinical Coding, and Statistics functions.
Monitor departmental workflows to ensure efficiency, productivity, and service quality.
Ensure timely retrieval, filing, scanning, storage, and disposal of medical records in accordance with hospital policies.
Monitor departmental quality objectives, key performance indicators (KPIs), and service standards.
Ensure confidentiality, security, and integrity of patient information.
Review and monitor requests for release of medical information in accordance with legal and hospital requirements.
Support implementation of information governance and data protection initiatives.
Prepare departmental reports, presentations, and management updates.
Coordinate HIMS Committee meetings and HIMS departmental meetings.
Present performance data, quality indicators, and improvement initiatives during meetings.
Maintain documentation and records of departmental activities.
Assist in checking return of medical record folders from wards and movements are accurately documented and tracked.
Review, check, and validate medical records, discharge summaries, and related documentation to ensure accuracy, completeness, and consistency.
Conduct administrative audits and produce reports on the completeness, legibility, and accuracy of medical record documentation.
Prepare, organize, and submit monthly E-Reporting and SMRP data to the Ministry of Health (MOH), mortality statistics to Group Clinical, KPJHQ and hospital management within the timelines given.
Provide and assist in the preparation and submission of coding-related reports, summaries, and analyses for state district center and hospital management use.
Gather, collect, and dispatch the external form from the nearest JPN branch within the stipulated time.
Indent and collect the external form from the nearest JPN branch to ensure availability.
Liaise and communicate with medical practitioners to clarify incomplete, uncertain, or unclear documentation in patient medical records.
Liaise with consultants, clinical departments, insurance companies, legal firms, government agencies, and external stakeholders regarding HIMS-related matters.
Perform any other responsibilities assigned by immediate superior or hospital management.
3) JOB REQUIREMENT
Diploma in healthcare management or bachelor's degree in health administration or other relevant qualifications.
Certified Clinical Coder or equivalent credential preferred.
Minimum 1–2 years of experience in Health Information Management, Medical Records, Clinical Coding, or healthcare administration. (fresh graduates may be considered).
Knowledge of PDPA and International Classification Diseases (ICD 10 & ICD 11).
Familiarity working in HIMS / medical records.
Experience of electronic medical records / hospital information system (HIS).
Strong analytical and problem-solving skills.
Detail-oriented with a focus on accuracy.
Proficiency in Microsoft Excel and PowerPoint.
