Job description

SUMMARY: Under the general supervision of the Asthma Program Coordinator, provides educational services, including community outreach, in-hospital classes and screening programs. RESPONSIBILITIES: Participates with development, planning and coordination of pediatric asthma education and screening programs. Develops educational materials as needed for professional organizations or special patient groups. Translates information as necessary. Schedules appropriate staffing for programs and orders necessary supplies. Provides pediatric asthma education programs consistent with ability, education and training in schools, hospital clinic and other partner programs. Provides home visits if needed for follow-up on Asthma education for those to families with special needs. Evaluates environmental triggers and provides trigger control strategies. Initiates follow-up for appropriate program participants with abnormal asthma surveys and/or no pediatric primary care provider. Assures contact with such individuals until appropriate care is provided. Provides participant follow-up reports as directed. Identifies areas where program recruitment is necessary and works with Program Director to increase referral base. Acts as a resource to parents experiencing barriers to accessing care. Makes referrals to appropriate resources and facilitates the referrals as needed. Works with Hospital departments, social service agencies, physicians and community groups to eliminate barriers for individuals at high risk and initiates programs where gap areas are identified. Participates in meetings, quality improvement teams and other such committees as required. Conducts program evaluation by collecting data, conducting designated chart reviews and documenting delivery of interventions. Ensure subject confidentiality. Provides monthly progress reports of activity including meetings, classes led and clinics attended. Maintains quality assurance, safety, environmental and infection control in accordance with established policies, procedures and objectives. Works with partner agencies in the community to promote project goals. Performs other related duties as required.

MINIMUM QUALIFICATIONS

BASIC KNOWLEDGE: Bachelor’s Degree in a healthcare related field is required, or equivalent years of relevant experience may be considered at the discretion of the Department. Certification as an Asthma Educator (AE-C) by the National Asthma Educator Certification Board (NAECB) required. Bilingual English/ Spanish preferred

EXPERIENCE: One year related and progressively responsible clinical experience; basic computer skills. Teaching experience preferred. Experience in dealing with high-risk populations. Prior experience using an electronic health record preferred.

INDEPENDENT ACTION

Performs independently within department policies and practices. Refers specific complex problems to supervisor where clarification of departmental policies and procedures may be required.

Pay Range: $26.80-$44.21
