Nurse Health Advisor

Under strict supervision, the Nurse Health Advisor collaborates with the primary care team to provide preventive care services that aim to enhance health outcomes and bridge deficiencies in care delivery, in adherence to the principles of Value Based Care. This position involves assisting in the formulation of tailored care plans and guiding patients towards adopting healthier habits through behavioral modifications.

Essential Job Function

• Provides support in identifying eligible patients for Medicare Annual Wellness Visits and Chronic Care Management (CCM) and recording care data within the Electronic Medical Record (EMR).
• Teaches patients about improving health through changes in behavior and collaborating to establish care plans and treatment targets.
• Partners with the Care Guide in identifying and organizing patient schedules for screenings, examinations, and immunizations.
• Aids in improving quality by examining information, such as evaluations of interventions, to enhance the quality of care provided.
• Initiates conversations with patients before their appointments to organize pre-visit arrangements and recognize areas requiring attention in their care.
• Supports in identifying individuals from Triad HealthCare Network (THN) and other vulnerable demographics who have been admitted to the hospital, to facilitate communication about inquiries and upcoming check-ups.
• Executes any other responsibilities as delegated.

Education

• Required: High School Diploma or equivalent, Graduate of a Licensed Practical Nursing program.

Experience

• Required: 3 years of related experience.

Licensure/Certification/Listing

• Required: LPNBLS (CPR)-American Red Cross or AHA Healthcare Provider.
