Job description
Overview Provides assessment and treatment of patients requiring physical therapy services within the Home Care setting. Is a resource to the Field Staff in the areas of community rehabilitation and individualized rehabilitation needs of the patient. Participates in the utilization review, quality improvement and interdisciplinary conference of the organization. Qualifications Age of Patients Served: Pediatric (1-12 years) Adolescent (13-17 years) Adult (18-64 years) Geriatric (=>65 years) Required Education: Bachelor's degree. Specific course work: Physical Therapy preferred Necessary Skills: 1. Competency in the provision of physical therapy. 2. Excellent written and verbal communication skills. 3. Knowledgeable in patient assessment. 4. Demonstrate age-specific competency. Required Licensure/Certifications: - Licensed to practice Physical Therapy in the state of Florida, - CPR certified. Required Experience: Minimum of one year Physical Therapy experience. Supervision: Supervisory responsibility - In-home supervision of LPTAs and Home Health Aides if patient is not receiving nursing services.
