As a Hospice Admission Nurse (RN), you will:

  • Conduct holistic assessments addressing physical, emotional, psychosocial, and spiritual needs

  • Educate patients and families about hospice philosophy, services, and care expectations

  • Coordinate hospice admissions, consents, and transitions of care

  • Establish and communicate the initial hospice plan of care

  • Collaborate with physicians, interdisciplinary teams, and hospice leadership

  • Provide functional supervision to LPNs and Hospice Aides

  • Document accurately and in a timely manner to support continuity and quality of care

  • Participate in admissions and on‑call rotations as assigned

This role offers a balance of autonomy, clinical judgment, and compassionate care.

Qualifications

  • Graduate of an approved nursing education program

  • Current Registered Nurse (RN) license in the state of Ohio

  • Minimum of one (1) year of acute care nursing experience

  • Hospice or home health experience preferred

  • Strong communication and interpersonal skills, including grief and loss support

  • Valid driver’s license and reliable transportation
