Schedule: PRN Shift : Days Location: 2350 MEADOWS BLVD, Castle Rock, 80109

The role you'll contribute: Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients; linkage to resources in the community.

The value you'll bring to the team: Assess patient's physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data. Evaluates patient's response and intervenes to ensure optimal patient outcome. Develop and implement patient plan of care and observe outcomes in accordance with social work standards and in collaboration with the interdisciplinary team. Demonstrate competency in a variety of therapeutic/diagnostic interventions.

Qualifications The expertise and experiences you'll need to succeed: Master of Social Work (MSW) Current Basic Life Support (BLS) from American Heart Association
