The Community Health Advocate will assist members to locate and connect to needed services and resources in the medical system and community that will make it easier to manage their health. Partner with medical systems and community and assist members to problem-solve barriers to health conditions and navigate existing community and medical services. Identify resource gaps in medical and community services and support implementation of new services to close identified gaps.
You will focus on:
Assesses psychosocial and social determinants of health needs for low risk members and documents assessment results or augments available information in appropriate systems
Consults with or refers members to licensed staff (social worker, nurse case manager etc.) as required based on member social, health risk and medical complexity
Establish relationships and partners with community resources, health plans and providers by participating in community engagement activities with local agencies e.g. faith-based organizations, community centers, government agencies, parks, recreation centers and schools
Assists members with problem-solving barriers to health conditions by identifying, locating, connecting to and navigating needed community and medical system services, including visiting members, accompanying members to medical appointments and assisting members with completing forms to access needed services
Actively engages, builds rapport and establishes trusting relationships and RS23 facilitates collaborative communication with members and member family support systems
Identifies social determinants of health concerns/gaps, develops and documents a plan to address social and health disparities in accessing appropriate care and services
Documents member updates and progress notes in appropriate systems and submits timely reports
Actively participates in Interdisciplinary Team Meetings and reports updates on member needs and progress
Files and makes necessary follow-up on mandated reporting, e.g. Adult Protective Services, Child Protective Services, etc.
Other duties as assigned including occasional home visits as directed by Manager
The Skills, Experience & Education You Bring
3+ years in healthcare working directly with patients
Communication and ability to work with various members of the clinical team.
Electronic Health Record knowledge and ability to document following standards, policy and procedures.
Time management and ability to manage a case load with minimal supervision.
Medical assistant experience a plus.
The Base Salary for this position is $25.00 - $30.00/hr
