Community Health Worker / KYNECTOR
Full-Time / Non-Exempt Position Location: Paducah, KY Service Area: Purchase District Health Department’s service region Work Week: 37.5 hours Pay: $18-$21 an hour negotiable with experience.
Position Overview
Purchase District Health Department is seeking a compassionate and community-focused Community Health Worker / Kynector to support individuals and families in accessing healthcare coverage, diabetes resources, and community-based services throughout the Purchase District.
This position serves as a trusted resource for community members by assisting with Medicaid enrollment and renewal through kynect, providing education and support related to diabetes prevention and management, and participating in outreach initiatives across the region. The Community Health Worker / Kynector will work closely with community partners, healthcare providers, and public health staff to help connect residents with programs and services that improve health outcomes and reduce barriers to care.
This is a full-time, non-exempt role ideal for candidates with strong communication skills, a passion for helping others, and experience in healthcare, social services, community outreach, or public health.
Primary Responsibilities
Kynect & Medicaid Assistance
- Assist individuals and families with Medicaid applications, renewals, and health coverage navigation through kynect.
- Provide education regarding available healthcare coverage options and eligibility requirements.
- Help clients gather required documentation and complete application processes accurately and confidentially.
- Maintain accurate records, documentation, and case notes related to enrollment activities.
- Stay informed on kynect policies, Medicaid guidelines, and eligibility updates.
Diabetes Education & Community Support
- Provide basic education and support related to diabetes prevention, management, and healthy lifestyle practices.
- Connect clients with healthcare providers, diabetes education programs, and community resources.
- Distribute educational materials and promote awareness of chronic disease prevention services.
- Support individuals in overcoming barriers related to healthcare access, transportation, nutrition, and community services.
- May occasionally provide home visits to clients in need.
Community Outreach & Engagement
- Participate in community outreach events, health fairs, school functions, and public health initiatives throughout the Purchase District.
- Build and maintain positive relationships with community organizations, healthcare providers, schools, and partner agencies.
- Promote available public health services and assist with outreach efforts targeting underserved populations.
- Represent Purchase District Health Department in a professional and community-focused manner.
Administrative & Professional Responsibilities
- Maintain accurate schedules, reports, and program documentation in compliance with departmental and grant requirements.
- Assist with data collection, reporting, and tracking of outreach and enrollment activities.
- Attend trainings, meetings, and continuing education opportunities as required.
- Perform additional duties as assigned in support of departmental and community health initiatives.
What is a Kynector?
- A Kynector is a trained enrollment assister who helps individuals and families apply for and maintain health insurance coverage through kynect, Kentucky’s health benefit marketplace.
- Kynectors support Medicaid applications, renewals, eligibility questions, and overall healthcare coverage navigation.
- They serve as a trusted community resource, helping people understand their options and connect to the care and services they need.
Minimum Qualifications
Education/Experience
- High School Diploma or GED required.
- Associate Degree or higher in Public Health, Social Work, Healthcare Administration, Human Services, or related field preferred.
- Experience in community outreach, healthcare enrollment, social services, customer service, or public health preferred.
- Experience working with diverse or underserved populations preferred.
Skills & Competencies
- Strong interpersonal and communication skills with the ability to build trust within the community.
- Ability to explain healthcare coverage information and community resources in a clear and supportive manner.
- Strong organizational and documentation skills.
- Computer proficiency including Microsoft Office and web-based enrollment systems.
- Ability to maintain confidentiality and professionalism while handling sensitive information.
- Ability to work independently and collaboratively in both office and community settings.
Physical & Travel Requirements
- Must be willing to travel throughout the Purchase District region for outreach events, meetings, visits, and community activities.
- Must possess a valid driver’s license and reliable transportation.
- Occasional evening or weekend may be required.
- Must be able to lift up to 50lbs and be able to stand for short periods of time.
Pay and Schedule
- Full-time, non-exempt position with a 37.5-hour work week.
