Physical Therapist (5 Openings) – Home Health Team | Chicago, IL

What You Need to Know:

• Location: Chicago, IL 60601
• Full-time position (40 hrs/week) | Monday-Friday
• Salary: $55.00 per hour
• 1 weekend per month (Saturday & Sunday, day shift with scheduled patients)
• 2 holidays per year (1 summer / 1 winter)
• No on-call required
• Local travel to residential homes

Key Responsibilities:

• Perform Physical Therapy assessments and develop treatment programs
• Record and report patients’ reactions to treatment and condition changes
• Instruct patients and caregivers on therapy programs and device use
• Supervise Physical Therapy Assistants and, when appropriate, Certified Nursing Assistants
• Prepare reports and participate in staff and Agency development activities

• Valid Physical Therapy license
• Prior experience in home health or a similar setting preferred
