Social Worker MSW Full- time Day

Social Worker MSW at Providence Santa Rosa Memorial Hospital in Santa Rosa, CA. This position is Full-time and will work 8-hour, Day shifts. The Social Worker provides comprehensive services to include psycho-social assessments, community resource information and referrals to patients, their families and loved ones, ranging in age from infants through geriatrics, as it supports the continuum of care. Provides coordination of complex discharge planning needs, and facilitates multidisciplinary care team involvement to expedite plan of care for patients.
