In this dynamic leadership role, you will provide both clinical and strategic direction across our Nursing Service, working closely with multi-disciplinary teams to coordinate seamless, patient-centred care in people’s homes. You’ll play a key role in service development, collaborating with internal and external partners to ensure high-quality support for patients and their families.
As a senior leader, you’ll oversee team performance through regular one-to-ones, appraisals and development opportunities, while also leading on quality, safeguarding, and the management of incidents and concerns. You’ll be actively involved in key operational and clinical forums, including governance meetings, ensuring effective care planning and continuous improvement.
This is a fantastic opportunity to combine leadership, clinical expertise, education and service innovation—making a meaningful difference every day for those who need us most.
What you will be doing:
- Lead the delivery of high-quality, person-centred palliative care, enabling patients to remain in their place of choice.
- Coordinate care through close collaboration with multi-disciplinary teams, ensuring seamless support for patients and their carers.
- Provide strong clinical leadership and oversight, ensuring compliance with the NMC Code and competency frameworks.
- Manage and support the nursing team through effective leadership, workforce planning, performance management, and development.
- Build and maintain positive relationships with key stakeholders, including commissioners, GPs, hospices, and community partners.
- Oversee quality, governance, and safeguarding, including managing incidents, complaints, and driving continuous improvement.
- Contribute to service development and strategy, identifying opportunities for growth and ensuring services meet local community needs.
What we’re looking for:
- Proven leadership and management experience in a healthcare setting including community services, with a track record of leading, developing and supporting clinical or nursing teams.
- Qualified Registered Nurse with a relevant degree and current NMC registration.
- Strong background in service development, audit, and continuous improvement.
- Experience in risk management, with the ability to identify, assess and escalate concerns effectively.
- Confidence in managing complaints and driving learning to improve service quality.
- A collaborative and proactive approach, with excellent communication and leadership skills.
- Flexibility and willingness to travel regularly, including weekly travel to our hub at St John’s Hospital, Livingston.
