What We Offer Care Connections Specialist I (Fulltime, Onsite) Location: Novant Health Presbyterian Medical Center Schedule: Monday – Friday | 11:30am - 8:00pm Be the Bridge Between Emergency Care and What Comes Next At Novant Health, our Care Connections Specialists play a critical role in ensuring patients don’t leave the hospital without a clear path forward. In this onsite position, you’ll work directly alongside clinical teams—supporting patients in real time as they prepare for discharge from the Emergency Department. You won’t just schedule appointments—you’ll help patients understand why follow-up care matters and ensure they’re set up for success beyond the hospital walls. Why Join Novant Health? What You’ll Do Round on Emergency Department patients who are expected to discharge Engage patients at bedside to coordinate follow-up care before they leave the hospital Monitor and work from a continuously updated discharge report to prioritize patient outreach Schedule follow-up appointments with primary care providers (PCPs) or specialists within 7 days of discharge Coordinate care for patients both within and outside of the Novant Health network Review patient charts in advance to identify gaps, including patients without an established PCP Assist patients in establishing care with a PCP when needed Educate patients on the importance of timely follow-up care and how it supports their recovery Provide clear next steps and appointment details to ensure patient understanding and compliance Document all interactions accurately in the appropriate systems What We’re Looking For EPIC experience, highly preferred Healthcare appointment scheduling experience, highly preferred Education : High School or GED, required. Experience : Minimum six months experience in customer service or customer relations, required. Experience working in a call center or medical setting, preferred. Additional Skills (required): Excellent interpersonal and verbal communication skills to provide “remarkable” customer service. Requires excellent organizational, problem solving and critical thinking skills. Requires the ability to organize and prioritize work in a stressful environment, be detail-oriented, and have excellent time management. Must be able to interact with individuals of all cultures and levels of authority. Requires the ability to maintain confidentiality. Must be able to work independently within a team setting. Team player who is positive and self-motivated. Ability to successfully complete generic and department-specific skills validation and competency testing. Computer proficient in Microsoft Office, with the ability to multi-task, working in a variety of software and internet platforms while conversing with a consumer and documenting information. Ability to use a variety of software packages to produce correspondence and documents and maintain presentations, records, spreadsheets and databases. Active learner able to adapt priorities in a change-ready, fast-paced environment. Additional Skills (preferred): Knowledge of basic medical terminology preferred. Knowledge can be obtained through formal classes or work experience. Working knowledge of electronic medical record systems preferred.
