Job Details

The Community Health Worker (CHW) serves as a bridge between healthcare providers, social services, and the community, with a focus on improving health outcomes for individuals, families, and communities. CHWs provide essential support through outreach, health education, resource coordination, and advocacy, particularly for underserved populations. CHW will provide social support, complete Health Related Social Needs (HRSN) screening, informal counseling, and advocate for individual and community health needs. Home and community-based visits are work assignment specific but could be an essential function of a CHW's work. This role requires cultural competency and a commitment to empowering individuals to improve their health and well-being. Location assignment and work settings could include: outpatient care areas, patient homes, inpatient settings, skilled nursing, community organizations, and participating in community events. Participation in community events and some evening/weekend hours may be required.

Job Description

Job Duties

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Manages caseload of patients/community members and completes outreach and follow up as needed
  • Assists in developing and implementing care plans in cooperation with patients/community members, families, and healthcare team and completes accurate, timely documentation.
  • Responsible for building trusting relationships with patients/community members and their families
  • Identifies social needs through HRSN screening and provides guidance, supports enrollment and navigate eligible individuals in federal, state, and local private or nonprofit/social service, health and human services programs
  • Provides culturally and linguistically appropriate health education materials within the scope of training and promotes healthy behavior change
  • Assists patients and families in effectively communicating with their health care team, understanding and carrying out their healthcare providers' advice, and communicates to care team any changes in plan or medical condition


Minimum Qualifications

  • High School Diploma/GED
  • Connect with patients/clients/families and community members by staying open-minded and having empathy and compassion
  • Engage in active listening and be adaptable to the needs of the people and team members the CHW is supporting and collaborating with
  • Help patients/clients/families understand their needs, overcome barriers and connect to resources within the community


Additional Qualifications


    Physical Demands
    Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
