JOB SUMMARY

The Community Health Worker Specialist (CHW) will provide outreach, coaching, and health related social support for patients of all ages. This position will focus on reducing barriers to healthcare services through providing and assisting patients in finding social support services and resources, care coordination, and promotion of health and wellness principles. They will partner with Care Coordination RNs with Care Management services

This role may provide basic education on wellness, mental health, brief coaching and discreet skill building, and coordination in accessing support to address identified needs. Furthermore, the CHW will foster collaborative relationships with other providers and social support agencies, government institutions, and other organizations to ensure ease in coordination of care across service domains.

In addition, the CHW will assist the population health program in patient outreach, program coordination, and health initiatives to improve health and decrease care gaps.  The CHW will also need to be available for mobile visits to patients’ homes to complete assessments and provide other services in the home setting.

JOB SPECIFIC DUTIES AND RESPONSIBILITIES

  • 1. Provide ongoing outreach, coaching, and health related social support for patients of all ages.
  • 2. Provide basic education on primary care and mental health and wellbeing, discrete skill building, and coordination in accessing community supports to address identified needs.
  • 3. Foster collaborative relationships with providers and other Summit Pacific Medical Center (SPMC) caregivers and other community organizations which support our patients. 
  • 4. Appropriately document and submit charges for services provided.
  • 5. Must have the ability to drive and care for patients off- campus and meet the driver qualifications per policy in order to perform home visits, outreach to other community organizations, and attend non-SPMC appointments with patients as appropriate.
  • 6. May offer additional support and services within the scope of their job description and role.
  • 7. Coordination and recruitment for population health programs.
  • 8. Assist patient coordinators with patient outreach, scheduling Annual Medicare Wellness Visits, and initiatives to improve health and decrease care gaps
  • 9. Performs special projects and other related duties as assigned.

ORGANIZATIONAL RESPONSIBILITIES

In addition to the duties and responsibilities listed above, all employees are expected to support the Summit Pacific Medical Center mission, vision and values; comply with SPMC policies and procedures; and conduct themselves in an ethical, professional, respectful, and collaborative manner at all times.

REQUIRED EDUCATION AND EXPERIENCE

  • Lived experience and/or understanding of the greater Grays Harbor Community.
  • A minimum of 3 years of relevant work experiences in healthcare, social services or similar fields.

REQUIRED LICENSES, CERTIFICATIONS, AND/OR REGISTRATIONS

  • 1. Completion of a SPMC approved CHW training program within 6 months of hire if not previously completed.
  • 2. Valid WA driver’s license with a satisfactory driving record; No more than two (2) moving violations within the past two (2) years.
  • 3. Defensive Driver Course specified by SPMC required within 6-monhts of hire.
  • 4. Drivers must be a minimum of 21 years of age.

REQUIRED KNOWLEDGE, SKILLS, ABILITIES

  • 1. Strong organizational skills: ability to prioritize, and reprioritize, work as needed to meet objectives.
  • 2. Professional and effective written and verbal communication skills. Ability to identify and employ communication strategies appropriate to the audience. Grammar and punctuation skills as needed to compose professional documents.
  • 3. Proficient in the use of current technology and software, including intermediate to advanced Microsoft Office skills (Word, Excel, Outlook).
  • 4. Proven ability to handle confidential information with discretion. Ability to develop and maintain accurate documentation and records.
  • 5. Demonstrated teamwork and collaboration skills. Strong customer service and interpersonal skills; ability to maintain composure and interact calmly and professionally with co-workers, providers, patients and visitors in a fast-paced, high-stress environment.
  • 6. Ability to work independently using good judgment, problem-solving, organization, and prioritization skills to complete assignments in a timely, accurate, and effective manner.
