Social Worker – Memory Care

Our Social Workers provide long term wrap around support for patients with dementia throughout Western WA. Our Social Workers work side by side with Primary Care clinicians to make huge impacts and improve patients’ lives.

Our Memory Care Social Workers

•

Facilitate care which will assist patients and families with maintaining or restoring health and wellness or supporting a dignified death

•

Implement and monitor the plan of care

•

Evaluate the response of individuals to interventions and assume responsibilities for the analysis and modification of the plan of care consistent with intended outcomes

The ideal candidate would have a passion for memory loss care, and palliative end of life care.

Minimal qualifications

•

Education: Bachelor’s or master’s degree from a CSWE accredited school of Social Work

•

Experience: 3 years of progressively responsible and directly related work experience in a healthcare setting required.

•

Certification/License: Case management certificate preferred, mandatory after two years of employment.
