Under minimal supervision, the Clinical Pharmacist, Quality is responsible for meeting Quality (Hedis/Stars) based clinical patient care goals for value-based patients in the Washington region. The Clinical Pharmacist, Quality is responsible to follow processes to improve patient care, outcomes, and quality improvement metrics related to CMS HEDIS/Star ratings. This care involves multi-disciplinary interaction with patients, pharmacists, physicians, nurses, and other health care providers in a dynamic environment to provide information and education about the safe and effective utilization of medications.

ESSENTIAL DUTIES & RESPONSIBILITIES
90% Clinical Pharmacist
- Reviews and provides medication reconciliation and recommendations to optimize medication therapy to prescribers to include gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution and generic substitution.
- Assess risk vs. benefits of medication therapy based on individual patient factors to include social determinates of health.
- Utilize evidence-based practice to promote quality patient care, protocol development, care process model development, and best practice standards for assigned patient population.
- Complete discharge medication reconciliation and provide counseling to patients transitioning from hospitals, SNFs, institutions or other high-risk patients.
- Provide medication therapy management (MTM) services for identified patients
- Provides medication optimization and quality improvement interventions for patients to improve medication 5 Star and HEDIS measures.
- Participates on interdisciplinary care teams to support medication recommendations for high risk members.
- Document patient and provider encounters in the appropriate electronic health record, through fax, text or by telephone.
- Communicate with patients, pharmacists, care teams, and doctor’s offices to resolve medication issues or communicate recommendations as necessary.
- Participate and lead committees, task forces and projects as appropriate.
- Develop and participate in ongoing reviews of policies and procedures, protocols, guidelines and tools to improve member medication management.
- Serves as clinical resource and consultant for medication utilization, medication management, quality and safety for patients, teammates, and network groups.
- Knowledgeable of current and emerging relevant regulatory requirements and clinical updates.
- Active participation in various clinical and regional departmental meetings and forums to improve clinical programs, enhance processes, and share clinical information.
- Continuously look for ways to streamline and improve workflow.
- Precept, train and/or mentor teammates, providers, interns and pharmacy residents as needed.
- Supports medical management, care management, and pharmacy quality initiatives.
10% Performs other duties as assigned
MINIMUM QUALIFICATIONS
Education : Doctor of Pharmacy; ASHP Pharmacy Practice Residency required or Pharmacy Board Certification.
Experience : Minimum of three to five years clinical pharmacy and/or clinical health plan experience in working with physicians and other health care professionals required. ASHP accredited residency or Pharmacy Board Certification preferred. Experience with Hedis/Stars measurement-based performance preferred.
Certificate/License : Must be a registered pharmacist in the state of Washington. Must be able and willing to obtain Oregon pharmacist licensure.
