Team Lead

Position Title: Bayside Team Leader Position Type: Full Time; 35 Hours/Weekly Benefits Eligible: Yes Location: Toms River, NJ Department: Bayside Salary: $42,000 - $47,000

Responsibilities

• Provide case management services to an assigned caseload, including follow-up and care coordination.
• Facilitate psychoeducational, skill-building, and rehabilitative groups for consumers and families.
• Facilitate individual and group supportive counseling sessions for program consumers.
• Complete psychosocial assessments for all new intakes
• Develop, implement, and review individualized treatment plans within required timeframes. Conduct outreach to consumers who miss scheduled program days through phone or face-to-face contact.
• Collaborate with external providers to support continuity of care and comprehensive treatment planning.
• Complete daily and weekly clinical documentation in compliance with agency standards and timelines.
• Participate in multidisciplinary treatment team meetings to support coordinated consumer care.
• Provide crisis intervention services and coordinate referrals to emergency psychiatric services as needed.
• Monitor medication appointments and renewal procedures to ensure continued consumer access to medications.
• Support and train newly hired case managers on program procedures and best practices.
• Participate in process improvement initiatives, committees, and assigned organizational projects.
• Master's or Bachelor's Degree in Social Work or related field
• LCSW/LSW Preferred
• Minimum of two years experience working with children in a mental health setting
• NJ Driver's License with less than 6 points
