Full-Time Social Worker (Subacute Unit) Monday–Friday | 8:30 AM–5:00 PM
Glacier Hills is seeking a dedicated Full-Time Social Worker to support our 44-bed subacute unit , with a primary focus on discharge planning and care transitions . This role is essential in helping residents move safely and successfully to their next level of care.
About the Role
As a Social Worker, you will play a key role in coordinating timely, patient-centered discharges. Responsibilities include:
- Leading discharge planning for subacute residents, ensuring safe transitions to home or other care settings
- Collaborating with interdisciplinary teams to coordinate services, equipment, and follow-up care
- Engaging residents and families early to establish discharge goals and expectations
- Identifying and addressing barriers to discharge (social, financial, or environmental)
- Conducting psychosocial, cognitive, and mood assessments to guide care plans
- Educating residents and families on Medicare, Medicaid, and post-acute care options
- Connecting individuals with community resources and support services
- Maintaining accurate, timely documentation •
About You
- Active Michigan Social Worker license
- Bachelor’s or Master’s degree in Social Work or related field
- At least one year of healthcare experience; discharge planning experience strongly preferred
- Experience in subacute, rehab, or long-term care preferred
- Strong communication, organization, and care coordination skills •
