Job description
Help residents thrive—every shift.
At Cappella Pueblo West, residents are not just receiving services—they’re citizens of a vibrant place to live, grow, and belong. We’re building a culture of thriving and citizenship where every person has purpose, voice, and choice. If you’re compassionate, detail-oriented, and energized by meaningful relationships, we’d love to meet you.
Position Summary
As a PRN Personal Care Partner / QMAP, you’ll step in to support day and night coverage as needed. You’ll meet daily personal care needs in a way that reflects our mission and values; anticipate, identify, and report care needs; and supervise and administer prescribed medications under the direction of a licensed nurse—following the “five rights”: right resident, right dose, right time, right medication, right route.
How You’ll Help Residents Thrive
Champion daily living with dignity: Provide timely assistance with ADLs—bathing, dressing, grooming, oral hygiene, toileting, ambulation, laundry, and mealtime support—tailored to each resident’s preferences (hair combed, shoes tied, etc.).
See the person, not just the task: Anticipate and identify changing needs (e.g., hygiene, broken glasses, skin concerns, hearing aids, supplies) and report promptly.
Be a steady, trusted presence: Maintain open, respectful communication with residents, families, and teammates; respond to family questions with warmth and professionalism.
Promote safety and wellbeing: Monitor resident status and environment; document observations as directed; complete work orders for repairs or replacements.
Uphold rights and protections: Immediately report suspected abuse, neglect, or misappropriation per regulation.
Support medication citizenship:
Review MARs and administer medications accurately and on time, in accordance with state law and community policy.
Document thoroughly, including refusals; restock medication carts/drawers with appropriate non-med supplies (gloves, cups, sanitizer); alert the RN/LPN or supervisor to any discrepancies, concerns, or supply issues.
Grow our shared culture: Participate in monthly staff meetings and contribute ideas that advance resident life and departmental excellence.
Pitch in where needed: “Other duties as assigned” to support a thriving Community.
What You’ll Bring
A heart for service and a belief that older adults are citizens with purpose and voice
Experience in senior living, home care, or healthcare support (preferred)
Current state-approved medication aide/assistant certification (as applicable)
Familiarity with MARs and medication administration procedures; unwavering attention to detail
Ability to document clearly, follow care plans, and communicate changes to nursing leadership
Reliability, teamwork, and respect for confidentiality and resident rights
Flexibility to accept PRN shifts across days, evenings, nights, weekends, and holidays
Why You’ll Love Working Here
Culture of Thriving & Citizenship: We elevate resident choice, community rituals, and meaningful roles—not just routines.
Team That Has Your Back: Collaborative leadership, ongoing training, and real-time support.
Impact You Can See: Go home knowing your presence made someone’s day better.
Apply today with your resume. Tell us how you’ve helped others feel at home, heard, and valued—because that’s what citizenship looks like in our community.
