About the Role
As a Hospital Liaison Worker (In-Reach), you will play a key role in providing specialist assessments, brief interventions and discharge planning for patients presenting with drug or alcohol related needs. Working in partnership with Dorset County Hospital and the Addictions Team, you will help ensure safe, coordinated transitions from hospital to community treatment to reduce relapse and prevent hospital readmission.
What You’ll Do
Provide in-reach support to patients within the hospital setting, completing assessments and appropriate interventions.
Co-develop multi-agency discharge plans and care pathways to support sustained recovery.
Work closely with hospital teams including Psychiatric Liaison, Emergency Department, Maternity and Addictions Services.
Maintain effective links with external services such as GPs, community treatment, outreach, housing, prison and justice services.
Offer advice, guidance and training to healthcare professionals on recognising and managing drug and alcohol related issues.
Contribute to improved treatment outcomes, quality of care and patient experience.
To Succeed in This Role, You’ll Need:
Experience working in substance misuse, mental health, or a related health/social care setting.
To be able to communicate confidently and builds strong, collaborative working relationships.
Understand the challenges faced by individuals affected by drug and alcohol use.
Be passionate about supporting people to make healthy changes and improve their quality of life.
Please note: The salary listed reflects the full earning potential for this role. Starting salaries depend on experience and progression within the band.
