- Job Summary:
- The Medication Listing Assistant is a certified Pharmacy Technician who supports pharmacy services by obtaining medication histories for patients that will be admitted to the hospital. Information is obtained by interviewing the patient, patient family, calling pharmacies, long-term care facilities and/or other providers. The information obtained is communicated to pharmacists and health care providers to determine treatment plans and medication orders during hospitalization and post discharge from the hospital.
- Works as part of a larger care team based outside the pharmacy, providing coverage for both direct admission patients and the Emergency Department
- Schedule – 7on/7off schedule, 1800-0430, Wednesday - Tuesday
- Responsibilities
- Review, clarify, document and verify home medications with the patient or utilize alternate sources for verification such as patient’s family, caregiver, pharmacy or medical provider within 24 hours of admission
- Use high-level communication skills to perform a patient interview and obtain a complete home medication list.
- This includes contacting retail pharmacies or physician offices to verify the medication history with persistence to achieve the highest level of accuracy.
- Ensure complete and accurate documentation for each identified medication in the electronic medical record with careful attention to detail.
- Discuss any identified issues related to the medication history with the clinical pharmacist team to promote high-quality transitions of care.
- Experience:
- Minimum of 3 years retail pharmacy experience needed
- Hospital experience (in addition to retail experience) preferred
What We're Looking For
- Education :
- High School Diploma or GED, required.
- 2 Year / Associate Degree, preferred.
- Experience :
- Acute: Minimum of two pharmacy skills (examples may include medication distribution, sterile compounding, and/or medication listing activities) required. OR...hired as a Pharmacy Technician 1 Certified and now competently performing a second pharmacy skill as described above. OR pharmacy intern enrolled in an accredited pharmacy school, required.
- Outpatient Oncology: Minimum of two years of experience in any setting required with one year of experience in sterile compounding (of IV and/or chemotherapy admixture), required.
- Specialty Pharmacy: Minimum of two years in any setting required with one year in specialty pharmacy setting, required.
- Licensure/Certification:
- Current registry with applicable State Board(s) of pharmacy, required.
- PTCB or NHA Certification within 60 days of hire, or proof of enrollment in pharmacy school/(NCBOP pharmacy intern license)/or FPGEE/FPGEC, required.
- A minimum of one relevant specialized certificate/certification from approved vendor such as PTCB, BPTS or NPTA (examples include but are not limited to product verification, medication history, hazardous drug management, billing/reimbursement, controlled substances diversion prevention, medication therapy management, sterile compounding). This must be obtained within 12 months of hire and maintained as applicable. Not applicable if currently enrolled in an accredited school of pharmacy with an active NCBOP pharmacy intern license.
- Additional Skills (required):
- State Registration and/or evidence of renewal is required for Technicians fifteen days prior to expiration and Technicians are required to be either certified through Pharmacy Technician Certification Board (PTCB) or National Healthcareer Association (NHA), or be actively enrolled in an accredited pharmacy school with an active NCBOP pharmacy intern license, or be active in Foreign Pharmacy Graduate Equivalency Examination (FPGEE) or Foreign Pharmacy Equivalency Certification (FPGEC).
- Considerable knowledge and application of basic math skills, including ratio and proportion relationships; strong verbal and written communication skills, ability to follow verbal and written instructions. Basic technology skills. Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
- Additional Skills (preferred):
- Considerable knowledge of medication names (both trade and generic) and their doses, routes of administration, and adverse effects; the ability to demonstrate patient interview skills and strong written and verbal communication skills with other healthcare professionals. Experience with inventory management.
