The Role Lead AI-enabled care at a dedicated SNF site. As a full-time Circadia NP, you will be the clinical anchor of one facility — conducting comprehensive E&M visits, initiating and adjusting plans of care, acting on real-time AI predictive signals, and building a culture of proactive intervention that measurably reduces preventable readmissions. You are not a consultant passing through; you own the clinical outcomes of your site.
What You’ll Do
- AI-driven patient prioritization: Begin each day with a Circadia dashboard review — triage overnight alerts, confirm your Tier 1–4 priority queue (new admissions → AI flags → change-of-condition → high-risk chronic), and align with the nursing team before first rounds.
•
Full-scope E&M visits: Conduct comprehensive assessments, initiate or adjust plans of care, place orders, reconcile medications, and document medical necessity — 20+ visits per day across new admissions, Circadia-flagged patients, COC escalations, and programmatic rounds.
•
Programmatic clinical rounds: Own four structured programs — Cardiopulmonary Monitoring Program (CMP), High-Risk Patient Management (HRPM), Post-Hospitalization Follow-Up Program (PHFP), and AI-Triggered Rapid Response — each with defined visit cadences and contribution targets.
•
Circadia platform integration: Reference AI trend data in every encounter note; confirm flag resolution in the platform; maintain a flag-to-visit conversion rate that supports medical necessity documentation.
•
Facility workflow ownership: Partner with DON and nursing leadership on daily huddles, COC escalation protocols, and new admission workflows; serve as the on-site clinical resource for AI-platform questions and staff coaching.

Documentation excellence: Complete all E&M notes using AI-assisted documentation tools within 24 hours; ensure CMS compliance for SNF billing including skilled care rationale and NP scope of practice standards.
•
Quality & continuous improvement
Participate in monthly internal chart audits, track facility-level KPIs (readmissions, fall rates, COC response times), and surface field insights to clinical ops and product teams.
How Success is Measured (KPIs) •
Visit Volume: 20+ documented E&M visits/day; target ~400+ visits/month

AI Alert Response: ≤ 24-hour flag-to-visit conversion; ≥80% Time to Acknowledgement Rate

Documentation: 100% of notes completed within 24 hours of each encounter using AI-powered tools
•
Readmission Impact: Demonstrated trend toward reduced 30-day readmissions, ED visits, and transfers over time
- Facility Adoption: Consistent Circadia workflow adherence and nursing team engagement; NP-led daily huddle participation
Must-Have Qualifications •
Licensed & certified NP in TN — ANCC (or equivalent) in Adult / Family / Geriatric / Acute Care
•
Full practice authority: able to initiate and adjust plans of care, place orders, and practice independently per TN scope of practice
•
Medicare eligible: company bills on your behalf; must be eligible for Medicare participation
•
2+ years NP experience with complex adult populations in SNF, LTC, or post-acute settings
•
Thrives in a startup environment: comfortable with evolving protocols, iterative workflows, and rapid growth
•
English required
Nice-to-Haves •
Tech-forward: proficiency in Google Suite, multiple EMRs, Slack, AI transcription/documentation tools
•
Geriatric or cardiopulmonary focus: experience with CHF, COPD, CKD, or complex wound management highly valued
