Hospice RN Case Manager - Twin Falls, ID

A community hospice provider in the Twin Falls area is hiring an RN Case Manager to support patients and families through high-touch, compassionate end-of-life care. This is a field-based role with a steady local territory, strong benefits, and a team structure designed to keep nurses supported in the field.

What you will do

  • Manage a hospice patient caseload across the Twin Falls/Magic Valley service area.
  • Complete skilled nursing visits in patients' homes and care settings.
  • Assess symptoms, coordinate medication and care-plan needs, and communicate changes quickly.
  • Educate and support patients, families, caregivers, and facility partners.
  • Collaborate with the interdisciplinary hospice team to deliver consistent, patient-centered care.
  • Document care accurately in CareEfficient or a similar EMR.

What we are looking for

  • Current RN license in Idaho, compact RN license, or a clear path to Idaho RN licensure before start.
  • At least 1 year of post-graduation RN experience. New grads are not a fit for this search.
  • Hospice, palliative care, home health, oncology, ICU, ER, med-surg, skilled nursing, long-term care, or another clinically intensive RN background.
  • Comfort working independently in the field while staying closely connected with the care team.
  • Strong communication with patients, families, physicians, facilities, and interdisciplinary teammates.
  • Reliable local travel in and around Twin Falls and the broader Magic Valley area.

Schedule and territory

  • Full-time RN Case Manager role based in Twin Falls, Idaho.
  • Field-based patient visits with geographically organized nurse territories.
  • One weekday on-call rotation and a weekend rotation generally every 4-6 weeks.
  • Two full-time on-call RNs are dispatched first; case managers provide backup coverage when needed.
  • Schedules are typically planned up to 3 months in advance.

Compensation and benefits

  • $80,000-$97,000 per year, depending on experience.
  • 100% employer-paid health insurance.
  • 401(k) match up to 3%.
  • Performance-based bonus opportunity after 90 days.
  • Mileage reimbursement at $0.47 per mile.
