Job description

The Post-Acute Coordinator is responsible for overseeing, executing, and verifying post-acute transition plans for patients. This role will address requests for post-acute services from both inpatient clinicians and community-based providers.  

  • High school diploma or equivalent is required.    
  • 2 yrs of an equivalent combination of education and experience in a hospital/medical related field is required.      

The Post-Acute Clinical Resource Specialist is responsible for the following:

  • Oversee, execute, and verify comprehensive post-acute transition plans for patients transitioning from acute care settings.
  • Address and process requests for post-acute services from both inpatient clinicians and community-based providers in a timely manner.
  • Coordinate patient placements with skilled nursing facilities, rehabilitation centers, home health agencies, and other post-acute care providers.
  • Communicate and collaborate with insurance companies and payers to obtain necessary authorizations and verify coverage for post-acute services.
  • Maintain accurate and detailed documentation of all patient transitions, including clinical summaries, care plans, and placement confirmations.
  • Collaborate with interdisciplinary healthcare teams, including physicians, nurses, social workers, and case managers, to ensure seamless care coordination.
  • Follow up on patient outcomes and post-acute care quality to ensure appropriate placement and continuity of care.
  • Ensure all transitions comply with regulatory requirements, accreditation standards, and organizational policies.
