Position Overview

This position facilitates admissions services by providing inpatient supportive care and seamless transition to home health/hospice. Assists the organization by assuring that the right care is provided at the right place and the right time. Positively promotes Sutter as the home health/hospice provider of choice within the communities for home health/hospice services. This position is responsible for attending hospital rounds, facilitating orders, assessing patient needs, Durable Medical Equipment (DME). This position enters referral into electronic medical record and completes referral process and admits patient. Serves as a resource by supporting the mission and vision of the organization.

JOB ACCOUNTABILITIES

Coordinates referrals to h ospice/home health.

Attends rounds, as appropriate, at hospitals/skilled nursing facilities to assist with identification of h ospice/home health patients.

Provides information about hospice/home health to patients/families.

Assesses patients for hospice/home health appropriateness, verifies orders, (i.e., wound care orders ostomy, etc.) to validate placement level of care needed and teaching that may need to be done with caregiver before discharge.

Obtains initial orders for comfort medications from hospitalist or p ain o bservation t ool physician, (Order initial medications from pharmacy.)

Enters referral into e lectronic m edical r ecord (EMR) and completes necessary documentation to support admission.

Coordinate communication and care needs with r eferral c enter supervisor or admission staff. Verify initial insurance authorization; notify patient of financial responsibility, and limitations of coverage, if any. Alerts hospice/home health of charity care need.

Obtains necessary consent and pertinent admission forms

Facilitates timely discharge from hospital/skilled nursing facility

Gathers discharge paperwork

Orders DME, medications, or arranges for other services as indicated.

Assist in arranging transportation for discharge home

Completes admissions and daily visits for g eneral inpatient c are (GIP) patients as directed by supervisor. Coordinate with i nterdisciplinary team.

Notifies hospital/skilled n ursing f acility billing of GIP level of care and billing procedures. Coordinate hospice/facility care of patients entering a hospital for non-terminal diagnosis. Inform hospital admitting of appropriate billing code and follow up with transfer information from hospice/home health.

Acts as liaison and problem solves with physicians and hospital staff.

Provides educational opportunities in hospital collaborating with r eferral c enter supervisor and h ospice/home health m anager.

Documents in hospice/home health EMR and hospital chart as appropriate.

Informs h ospice/home health s upervisor of quality issues.

Accepts assignments as designated from manager/supervisor.

Must be flexible with schedule including, but not limited to, the ability to participate in on call and/or department rotation for night and weekend coverage.

Quality Management (includes documentation and regulatory standards)

Coordinates and initiates documentation for hospice/home health

Correctly and clearly completes appropriate paperwork for admission

Provides accurate information related to insurance, medical group, and demographics

Documents information to track all activities

Develops and recommends policy and procedure changes as needed

Inform manager of quality issues

Collaborates with coworkers on staffing schedules

Fiscal Responsibility

Submits documentation in a timely manner and in compliance with relevant agency policies and procedures

Obtains supervisor approval for all overtime prior to the occurrence in accordance with agency policy and procedure

Accepts clinical assignments as designated by the supervisor

Practices effective time management techniques to meet productivity standards

Maintains positive professional relations with support personnel through appropriate and timely responses for information or paperwork necessary for the key clinical and billing functions of the agency Must remain current with all company communications

EDUCATION

  • Graduate from an accredited school of nursing

CERTIFICATION & LICENSURE

  • BLS-Basic Life Support Healthcare Provider
  • AUTO-Automobile Insurance
  • DL-Valid Drivers License
  • RN-Registered Nurse of California

TYPICAL EXPERIENCE

5 years recent relevant experience

SKILLS AND KNOWLEDGE

Possess critical thinking skills, in-depth knowledge of clinical disease entities common to hospice/home health and palliative care for terminally ill patient. Should also possess knowledge of hospice/home health reimbursement methods.

Must have the ability to function independently and as a member of an interdisciplinary team; ability to exercise independent, sound nursing judgment in planning and providing patient care.

Ability to recognize the special needs of h ospice/home health patients and others in the home.

Ability to communicate with patients and significant others in an effective, mature, caring manner.

Ability to maintain harmonious constructive relationships with internal and external customers.

Demonstrated effective written communication skills.

Ability to be aware of surroundings and respond to emergency and/or urgent events. Ability to select and utilize patient transfer/transport equipment safely.

Productivity standards – ability to meet productivity expectations.

Must be detail- oriented.

Pay Range is $71.52 to $95.12 / hour

The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health’s comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
