RN - Registered Nurse

  • RN Team Leaders assist in ensuring the general health and well-being of our residents by providing direct nursing care. Direct nursing care of residents encompasses planning, developing, organizing, implementing assessing, and evaluating the care of residents to ensure that the highest degree of quality care and regulatory compliance is maintained at all times.

RN - Admissions Coordinator

As an Admissions Nurse, you will be responsible for screening prospective residents for placement within the Elderwood organization. You’ll be the vital first point of contact—helping families and referral sources navigate care options while ensuring each prospective resident is appropriately evaluated for services.

You will bring a well-rounded understanding of Elderwood’s services, staffing, routines, and clinical operations. Your role is key in supporting census growth, timely admissions, and a smooth transition process for new residents

LPN - Licensed Practical Nurse

  • Licensed Practical Nurses (LPNs) are indispensable members of our interdisciplinary team and are charged with managing the daily clinical regimen of each resident and sub-acute patient. Our LPNs enjoy a unique opportunity to gain extensive clinical experience and provide advanced care in a home-like setting.

LNA- Licensed Nursing Assistant

  • Licensed Nursing Assistants (LNA) provide personal and nursing care to meet the physical and emotional needs of residents in Elderwood skilled nursing facilities (SNF). They are under the direction of a licensed nurse. State specific certification is required.

PT - Physical Therapist

  • Physical Therapy team members assist with ensuring the health and well-being of our residents by providing comprehensive and quality therapy services. This position is responsible for administering skilled physical therapy treatments ensuring that the highest degree of quality resident care is maintained at all times.

SLP- Speech-Language Pathologist

  • Speech-Language Pathologist team members assist with ensuring the health and well-being of individuals by making recommendations for a speech-language therapy treatment plan, if necessary, and carrying out treatments as planned; conducting an audiometric screening of resident upon request and making recommendations for further audiology service. This position ensures that the highest degree of quality care is maintained at all times.

  • As an Activity Leader, you will plan and lead a variety of fun and enriching recreational activities tailored to residents’ interests and abilities. Your efforts will contribute directly to enhancing quality of life and building community within our facility.

Floor Technician

As a Floor Technician, you play a critical role in maintaining the cleanliness, safety, and overall appearance of our 150-bed skilled nursing and subacute rehabilitation facility.

You will be responsible for routine and specialized floor care including scrubbing, buffing, stripping, refinishing, and extracting to ensure a clean, polished, and safe environment for residents, staff, and visitors. This hands-on role is essential to supporting a welcoming, home-like atmosphere and directly contributes to resident comfort, dignity, and safety.

Housekeeping Aide

  • Housekeeping team members assist with ensuring the health and wellbeing of our residents by providing housekeeping support. This individual is responsible for maintaining the cleanliness of all facility areas, including but not limited to, resident rooms, common areas and offices.

Maintenance Assistant

This dual role combines general maintenance responsibilities with safe resident transportation. As a Maintenance Assistant/ Transport Driver, you will support building operations by performing repairs, upkeep, and grounds maintenance to ensure a safe and welcoming environment. While also being responsible for safely transporting and assisting residents using company vehicles for admissions, medical appointments, and facility transfers.

Medical Records Coordinator

  • The Medical Records Coordinator (MRC) supports the Nursing Department in ensuring the health, safety, and well-being of residents by managing and maintaining accurate medical records. This role is essential in maintaining regulatory compliance, supporting census reporting, and contributing to quality assurance initiatives within our 150-bed Skilled Nursing and Subacute Rehabilitation facility.

Memory Care Program Specialist

The Memory Care Program Specialist provides therapeutic and social programming designed to meet the physical, cognitive, and emotional needs of residents living in our memory care community. Working under the direction of the Director of Activities, with support from the Director of Memory Care, this role plays a vital part in enriching daily life and promoting residents’ highest practicable level of well-being.

Unit Clerk

  • Unit Clerk team members assist with ensuring the health and well-being of our residents by providing administrative support. This position is responsible for coordinating proper placement of documents in Medical Record; performing secretarial duties for assigned unit; assist with inventory and requisitioning of nursing supplies, forms and stock medications; schedule various appointments, laboratory or other diagnostic needs.

Qualifications

  • Credentials verifying appropriate certificate and license in New York State- RN, LPN, LNA, PT, PTA; current State license/Certificate required.
  • Ability to communicate well verbally and in writing required.
  • Qualities of maturity, diplomacy, and ability to work well with others required. Caring, respectful attitude toward residents required.
  • Physical stamina for constant activity required.
  • Ability to perform tasks to established standards of excellence required.
