Description:
Home Health / Hospice Aide
Join Saint Agnes Home Health and Hospice, part of Trinity Health At Home, and make a meaningful impact by providing compassionate care to clients in their homes.
Responsibilities:
- Assist with personal care (bathing, grooming, dressing, mobility)
- Light housekeeping, meal prep, and laundry
- Monitor vital signs and report changes
- Support medication self-administration and equipment safety
- Maintain accurate documentation and communicate with RN supervisor
Qualifications:
- High school diploma/GED
- Must be a Certified Home Health Aide (HHA) in the state of California
- Federally compliant training and competency evaluation
- 1+ year experience in home care, hospice, or institutional setting
- Driver’s license and reliable transportation
- Ability to lift up to 35 lbs
