Location: UF Health Florida Recovery Center
4305 NW 90 Blvd. Gainesville, FL 32606
Schedule: Varies | 11:00am-9:30pm

Provides patient care services primarily to clients in the outpatient component (IOP, PHP and Community Housing) of the Substance Abuse Treatment Program, within an interdisciplinary team model. Promotes compliance with treatment plans and contracts. Facilitates patient participation in therapy and support groups and completion of assignments in the 12-step model. Is available to clients to deal with situations that may require attention prior to next treatment day. Monitors sub-lease living quarters for compliance with program and apartment complex rules including monitoring of living conditions and maintenance of equipment and supplies in the apartments. Facilitates new patient transition, drug screens and chart maintenance. Provides administrative and financial support to the Operations Manager as needed.
Responsibilities
- Facilitates patient activities per program schedule and contributes to the continuity of each patient's care as directed by the Operations Manager
- A. Facilitates partial hospital patient admission or transfer to PHP/IOP including orientation, documentation, and arranging for transportation.
- B. Facilitates transition to apartments including orientation, contract, and arranging transportation.
- C. Observes and completes random drug screens for PHP/IOP patients as needed and upon patients¿ return from pass.
- D. Reinforces client education: encourages client completion of assignments, attendance at and participation in therapy and support groups.
- E. Observes client responses and reports to the substance program team
- F. Promotes use of leisure activity facilities and other healthy life style activities in conjunction with recreation therapist
- G. Assists with Big Book Study, Community group or wrap-up group with inpatients in the evening
- Contributes to a safe and secure environment for clients, co workers by following established procedures and protocols.
- A. Observes for changes in client behavior, e.g. peer conflicts, missed groups, inappropriate comments, etc. and contacts therapist or nurse in charge.
- B. Contributes to the prevention of infectious diseases among employees and clients/patients by adhering to Infection Control Policies and Procedures.
- C. Reports all incidents and infection control variances to the nurse in charge. Completes employee accident form, as needed.
- D. Reports environmental safety hazards including damaged and broken equipment. Corrects or removes hazards.
- E. Responds to PHP client requests and needs in a timely fashion.
- F. Notes client's compliance with treatment contracts - individual treatment and apartment living.
- G. Initiates emergency measures as appropriate.
- Communicates effectively with interdisciplinary team and the with clients/family members to achieve desired outcomes.
- A. Verbal and written communication is clear and specific and purposeful.
- B. Completes appropriate forms thoroughly, accurately, and in required time frame.
- C. Utilizes therapeutic communication (protects clients dignity, listens actively, shows empathy, set limits etc.) when interacting with clients.
- D. Follows each individual's treatment plan - engages clients in scheduled activities to promote goal achievement
- E. Facilitates clients in their use of problem-solving skills and in positive interactions with peers and members of the treatment team.
- Contributes to and promotes the operational, financial and productivity goals of the assigned cost center and facility.
- A. Ensures procedures for transportation for PHP patients are documented and necessary gate cards are distributed and collected when discharged.
- B. Ensures UDS requisitions and results are documented and filed according to requirements for medical record and audit purposes.
- C. Completes paperwork for new PHP and apartment patients (identification, vouchers, keys, etc)
- D. Documents follow-up surveys in computer database.
- E. Provides support to Operations Manager with coordinating apartment contract and purchasing issues to ensure effective management of the program
- F. Operates and trouble shoots equipment, including those in apartment subleases. Supplies are monitored and ordered as needed and according to budget
- G. Provides administrative support to the Operations Manager regarding PHP and IOP program issues.
- H. Utilizes written and electronic/computer systems to facilitate client care and administrative responsibilities.
- I. Works as back up to financial counselor and works with financial counselor and Operations Manager to enable clients to maintain accountability in financial arrangements.
- Demonstrates self-development and accountability for job responsibilities.
- A. Adjusts work schedule to meet the needs of the program; stays in close touch with Operations Manager to address schedule changes.
- B. Accepts assignment and is flexible regarding changes in priorities and care needs.
- C. Participates in activities to increase knowledge and skills as well as complete mandatory requirements.
- D. Maintains accurate record of time worked and turns this is as required
Qualifications
Minimum Education and Experience Requirements
- High School diploma, or its equivalent required.
- College preferred.
- Previous but recent professional or volunteer experience in the substance abuse treatment field is preferred, including knowledge of treatment issues and relapse problems and a working knowledge of the 12 step model.
- Must have the ability to provide support and to set appropriate limits.
- The ability to communicate clearly both verbally and in writing is required.
- Computer experience including Microsoft Word and Excel and Organizational skills required.
- Financial counseling experience preferred.
- If in recovery must have maintained 1 consecutive year of sobriety immediately prior to and concurrent with employment.
Motor Vehicle Operator Designation: Employees in this position will operate vehicles for an assigned business purpose as a 'non-frequent driver'
Licensure/Certification/Registration: Substance abuse certification preferred.
