Location: UF Health Jacksonville Hospital
655 8th St W, Jacksonville, FL 32209
Schedule: Varies | 8:00pm-8:00am
Employment Type: PRN
Must be available to work weekends as needed to meet departmental and patient care needs.

Conducts mental health screening evaluations and determines and implements appropriate dispositions. Responsible for initial assessment of emergency room patients to include emergency mental status evaluations and dispositions recommendation assessments to identify appropriate resource utilization, discharge planning needs, mental health care needs and available related community resources.
Responsibilities
- Conducts assessments on individuals and documents all pertinent data according to Emergency Services policy and procedures.
- Determines and implements appropriate disposition/referral(s) and consulting as indicated, according to the needs identified during the evaluation process and according to procedures.
- Collects data as required for performance outcome measurements.
- Distributes Emergency Triage Unit Tracker daily every shift.
- Acts as advocate interpreting Baker Act to other agencies, family or concerned parties to assure conformity with patient's rights for both the patient's and staff welfare.
- Documents all consultations and data regarding disposition/referrals(s), including instructions to patient/family or others, appointments arranged, etc., according to procedures.
- Records all individuals seen for evaluation in the electronic Emergency Evaluation Log.
- Provides emotional support and crisis counseling to patients and families in emergency and urgent situations, and recommends appropriate levels of intervention to patient, family and treatment team as indicated.
- Assures the accuracy, completeness and confidentiality of the client's clinical record. Participate in data collection and research activities related to assigned clinical areas.
- Assures thorough and complete documentation including but not limited to biopsychosocial assessment and disposition/referral data, follow-up contacts or efforts, instructions to individual or family and their understanding.
- Assesses and monitors for risk, symptoms of trauma, and indications of abuse, neglect and/or abandonment. Uses appropriate reporting mechanisms.
- Documents or obtains documentation of past medical, psychiatric substance abuse and social history of each patient.
- Assures that any necessary consent forms are properly completed, signed and witnessed.
- Using Baker Act criteria and DSM V standards, evaluates and assesses any crisis telephone calls and/or walk-ins to determine need for services. Conducts emergency evaluations and/or arranges/refers for services as indicated.
- Acts as client advocate interpreting Baker Act to other agencies, family or concerned parties to assure conformity with client's rights for both the client's and staff welfare.
- Documents all consultations and all data regarding disposition/referral, including appointments arranged, etc., according to procedures.
- Performs other duties as assigned by management within job scope.
- Identifies/reports quality and risk management issues.
- Participates in staff meetings, self-directed work teams, and other departmental initiatives for performance improvement.
- Maintains communications with Psychiatrist team and Inpatient staff.
- Assists in the orientation of new personnel as assigned by supervisor
- Attend and participate in staff meetings in-service programs, related workshops, seminars, courses or other educational programs as requested or required
- Timely completion of required continuing education and/or hospital regulatory trainings, as assigned
- Assist in scheduling department personnel in order to maintain 24-hour coverage.
- Assist in covering department on all shifts as needed.
Qualifications
Required Education: Bachelor's degree
Preferred Education: Master's Degree in Social Work, Mental Health Counseling, Psychology, Nursing or closely aligned human services discipline.
Required Licensure/Certifications: Crisis Prevention Institute training or Aggression Control training is required within ninety (90) days of date of hire or transfer, and annually thereafter, for the Psych Unit.
Preferred Licensure/Certifications: LCSW, LMHC, LMFT, RNC, RN
Required Experience: Two to four (2-4) years' experience working with the chronically mentally ill population
Preferred Experience: Over 5 years working with chronically mentally ill population in emergency crisis setting.
Necessary Skills
- Ability to perform verbal and written communication.
- Ability to write proficient clinical notes
- Must be able to communicate and write English clearly, legibly and correctly at the college graduate level in order to make entries in the records of persons served.
- Documentation and paperwork must meet internal and external guidelines for content, accuracy and timeliness.
- Ability to recognize emergencies and life-threatening situations and respond appropriately.
- Ability to make clinical decisions affecting the lives of clients.
- Proficiency in EPIC, Microsoft Office Programs, Outlook, Excel and use of the Internet.
- Ability to work independently and manage time effectively to complete duties
- Must make sound decisions independently on a daily basis, including but not limited to completing tasks, recognizing emergencies, safety risks, and life-threatening situations and responding appropriately
- Must be able to interact appropriately with internal and external customers, including families, caregivers, community service providers, supervisory staff and other department professionals.
- Maintains working knowledge of departmental policies and procedures.
